Had this last night, in league semi-finals.
Was discovered on turn 5 by offending player who had had a good drive. I was the non-offending player.
Restarting the drive seemed like a waste of time, even though it would probably help me. We agreed to remove a player at random. I was going to let it drop at that
The offending player voluntarily forfeited a 50k card (it wasn't a particularly good one) and scored on turn 6 instead of stalling, which he easily could, as added concession. I did score in the first half to tie it up.

Had this on Cyanide. It seemed unintentional and neither party noticed, but it is a known bug and our rule states that the bug may not be used (regardless of intent). Since the drive it happened on caused a defensive TD, and having a 12th player was almost certainly of use in creating that TD, that particular TD was removed from the offending side and awarded to the other side.
Harsh? Possibly. But it will help prevent it happening again and, in an online league where it's impossible to otherwise intervene, it's as much so that people know to keep an eye out for their own bugs as anything else.

I had it happen to me in the championships of a very competitive league against a super competitive coach.
I struggled hard on my own offense. Really struggled. Finally managed to cause a KO. That made me look at the dug-out and notice that my opponent had had 12 players on the pitch. But now he had eleven. And league rules explicitly stated that that made it too late to call.
Goes without saying that I lost the match.
Honest mistake? I'll never know!
But given that the league rules for fielding 12 were quite severe, I doubt he would have gambled it.

In our league we have been using the 1 randomly selected player is sent off for the match (can be bribed). But like plasmoid said, if you notice when they are back down to 11 players it is too late.
I really wish that there were an official rule for this in the book!

In our league the rule is simple:
"the correctness of the set-up is responsability of both coaches"
If the opponent plays with 12 players it's his fault for doing so and your fault for not checking.
So he simply removes a player of his choice (with preference for standing, unmarked players).
Both players are supposed to collaborate towards a good game.
Harsh penalties encourage backstabbing attitudes (it's really just another kind of illegal).
We think a coach that can't count up to 11 does deserve more derision than protection.
