To many players on the field
OK this happened yesterday in one of our league games.
Starting the 3rd turn of the first half, both the coaches notice that one team started with 13 players on the field. The team that had the 13 had just lost 2 players and the coaches decided to overlook the problem and play on.
What should have been done?

The reason there's no official rules (before anyone says there should be) is because the rules say both coaches should count the players on the field, so as far as the rules are concerned, this error will never happen.

Currently an ex-Blood Bowl coach, most likely to be found dying to Armoured Skeletons in the frozen ruins of Felstad, or bleeding into the arena sands of Rome or burning rubber for Mars' entertainment.

Given that the team had lost two players perhaps this was the best solution, but generally my experience is that excess players are removed on a random basis. Sometimes just looking at linemen as most people accept that specialist players would have been included in the starting line up.

PS: Personally I normally find out I have too many players on the pitch after the opposition has scored, for some reason having extra players seems to be a hindrance not a help!

If everyone is really concerned about not having the right number of people on the field try this.
Take a permanent marker and write the numbers 1-11 in squares in each endzone. Then coachers line up their players across the numbers before transfering them to the field. That way, no one should have more or less than 11 players on the field.

My preferred solution is that the opposing coach removes excess players of his choice until you are down to the right number. So in this instance the opponent gets to remove 2 players. He could be nice and choose the two already off the pitch, or not and take the best two remaining players.
The reason I like this is that it makes about the penalty a little bit worse than the crime - so intentional fielding of extra players is very harshly dealt with, so encouraging both players to count.

Worst penalty would be to call 'illegal procedure' - ends the players turn (and gives you a RR?). Then you get your turn to smush players. If there's still 12 left, on opponents next turn--> Illegal Procedure...
Repeat until 11 players (or less) on pitch
