Change PO so the player has to choose whether to pile on before the AV roll, then add this to the skill description...
After the infamous Chaos All-Stars/Lowdown Rats match of 2467 where no less than 15 goblin players from the Rats were flattened into pancakes by the All-Stars infamous troll blocker Bork Bulge-Belly, the rules of the game were altered so that "Attacking a fallen opponent" included belly flopping onto them.
If the armour roll comes up a double (1,1 6,6 etc) then the piling on player is sent off, you may argue the call as usual.

I think that, if there really is a problem with Pile on, it is resolved enough by either one of these amendments.

We use this method in our league...gaining a possible 3+ on the armor roll should come with some risks (since you have to be prone to gain the benefits). The only tweak for P On is to move it back to choose before the armour roll.
Then its fine - there is no need to add a sending off chance - if you have this, Piling On will rarely, if ever be taken. Fouls tend to be performed by your average lino - so getting him / her sent off is no big deal. Losing your Norse Blitzer or Mummy IS a big deal - hence the skill will never be taken.

I think the reason we see so many piling on threads is that there is a real problem with it.
The players that wreak the most havoc with the skill are already throwing two dice blocks at your average player (black orcs, mummies) and so getting two advantages at once.
Personally, as a minimum, it needs to be changed to declaring before the armour dice are rolled. And it really needs more revision than that i think.
