Do you have an Apothecary? I'd take an Apothecary, and play on with the guy, just because Orcs don't accumulate a lot of injuries, and having a couple injuries just adds more flavor to the team. That said, in a competitive format, the optimal answer is probably to replace him now. You can do that and take an Apothecary.

Oh, and a NI on AV9 isn't nearly as bad as giving up free Mighty Blow. On 1296 perms:
